Position Purpose:

Consults and collaborates with interdisciplinary clinical and non-clinical support staff in the monitoring and evaluations of the quality assurance and risk management program activities at the veterans home.

Description of Duties:
Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor.

Interprets facility, departmental, state, and federal policies, regulations, and legislation for quality of patient care service in order to analyze and evaluate quality assurance and risk management data.

Consults and collaborates with interdisciplinary clinical and non-clinical support staff engaged in monitoring and evaluating quality assurance and risk management program activities to ensure the appropriateness and quality of patient care provided meets the regulatory requirements.

Identifies possible liability problems and issues of non-compliance with regulatory requirements.

Monitors, examines, and evaluates records of patient care.

Participates in in-house surveys with interdisciplinary clinical and non-clinical administrative and support services staff to identify potential problem areas. Recommends methods and actions to prevent and/or to correct staffing and operating deficiencies, and monitors follow-up activity reports to ensure timely and appropriate actions are taken to minimize and/or to prevent problem reoccurrences.

Participates in the organization and coordination of third party surveys conducted by certifying and accrediting agencies for compliance with State and federal regulations, legislation, and DHS standards by completing required pre-survey forms and assembling documentation and records for review by surveyors, and accompanying them on the on-site review, as required.

Investigates reported problems related to safety of patients and staff, injury reports and patient complaints and abuse. Recommends appropriate actions, provides guidance and assistance to facility staff to correct deficiencies, and prepares written and oral reports on problem/deficiency resolution.

Provides and assists in the implementation and monitoring of action plans to minimize liability problems and to correct deficiencies indicated by the independent survey team.

Develops systems such as quality assurance checklists and program observations.

Prepares reports and maintains files and records of quality assurance/risk management monitoring.

Participate in QAPI program/processes and on the facility QI Committee. Provide QAPI training as necessary to ensure staff are prepared to participate in QAPI. Consult with facility leadership, department heads, and PIP teams in the use of QAPI tools and techniques, data collection, and data analysis. Participate in promoting the use of Root Cause Analysis to identify causes and solutions to problems.

Performs work in accordance with the DMVA general safety rules and safety rules for this position. Immediately reports all workplace injuries or incidents to the supervisor; and/or immediately corrects any safety hazards in the work area or reports same to supervisor.

Use personal protective equipment when required such as gloves, masks, etc.

Performs other duties as required.
