Position Purpose:
Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. 

This position in the Division of Nutrition and Physical Activity, specific to asthma prevention and control (APC) organizational infrastructure, supports the Bureau’s mission of using current data and research, supporting community partners in implementing prevention strategies for chronic disease, injury, and violence to foster healthy and resilient communities across the Commonwealth of PA. This position focuses on activities to advance health equity and sustainability, strengthen partnerships, monitor data trends, evaluate data and facilitate strategic planning.

Description of Duties:
Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor.

Leads the development of program direction for assigned public health programs, including the development of goals and objectives.

Establishes and maintains effective working relationships with APC partners as well as other federal, state, and local partners, providers, and governmental entities.

Maintains a current understanding of the field of asthma, asthma prevention and management, social determinants of health, and health disparities to inform the development and implementation of targeted interventions.

Reviews policies and procedures and interprets and applies regulations and funder mandates to support job duties.

Supports and coordinates the development of grant applications where necessary to apply for funding to support asthma initiatives and strategies.

Prepares legislative analysis using appropriate DOH-formats to provide program’s position on legislation.

Recommends resource allocation and prepares funding proposals to support program goals and objectives.

Works with chronic disease epidemiology staff to identify asthma data trends, respond to data requests, and adheres to federal and state data management guidelines.

Provides direction for asthma data analysis and disseminates data to APC partners and the public.

Program Evaluation
Collaborates with funded entities and evaluation partner to evaluate the efficacy of asthma prevention and control initiatives.

Develops evaluation tools in collaboration with program partners and evaluator in order to support the evaluation of strategies.

Develops performance measures and target metrics in collaboration with program partners to monitor program outcomes and modify initiatives where necessary.

Implements evaluation plans in collaboration with program partners and evaluator in order to support the evaluation of strategies and documentation of program outcomes.

Assesses program outcomes to identify, recommend and implement methods for improvement and modification.

Evaluates policies or programs with equity tools to ensure that disproportionately impacted populations are identified, considered and included in policy or program implementation.

Communication
Coordinates with federal, state, and local partners to explain program requirements and accountability for evaluating and reporting process and outcome measures to federal and state agencies, community stakeholders, and advocacy groups.

Develops dissemination plans and products to share program outcomes to inform partners and stakeholders.

Prepares grant progress reports in accordance with funder requirements.

Educates partners, stakeholders and the public on asthma and evidence-based policy, system and environmental practices, strategies and interventions that can be implemented to prevent and control asthma.

Participates in meetings with funders to provide status updates and obtain technical assistance.

Prepares professional power points, talking points, reports and correspondence to communicate with partners, stakeholders and the public on asthma and related topics.

Serves on committees, boards, or task forces, as assigned, as the program representative for the department and to build relationships, identify opportunities for collaboration, share information and support program goals.
Oversees and facilitates the Pennsylvania Asthma Partnership.

Contract Management
Oversees selection and monitoring of grantees and contractors to support the implementation of assigned asthma prevention and control strategies.

Negotiates, administers and monitors grants and contracts for assigned asthma prevention and control strategies using status calls, monitoring reports, site visits and other relevant monitoring and evaluation tools.

Develops grants, contracts and MOUs for assigned asthma prevention and control strategies, including non-solicitation award documents, work statements, program narratives, budgets, and amendments in accordance with Department bulletins.

Evaluates program compliance using funder and Department guidance to ensure program goals and policies are being met.

Monitors invoices and expenditures in accordance with state and federal guidelines in order to ensure funds are being expended appropriately.

Maintains an ongoing relationship with contractors and grantees based on the goals of the ACP and funder requirements in order to implement program objectives.

Develops and recommends plans of correction for contracted entities and monitors implementation.

Performs other duties as assigned.

Public Health Emergency Response:
This position may be given an assignment to support the public health emergency response mission of the Department of Health. Emergency assignments could at times be received with little or no advanced notice, and may continue for any period of time, ranging from a few hours to several weeks, depending upon the nature of the circumstance. The emergency assignments may at times be performed under adverse conditions and may require travel to another site other than the employee’s assigned headquarters for periods of time.


Additionally, this position demonstrates awareness of the vulnerable populations the organization serves by identifying, providing, and advocating for resources, services, communication methods, and policies that would help those populations achieve health equity.





Decision Making:
