Position Purpose: Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. The Medical Certified Registered Nurse Practitioner (CRNP) Practitioner is an independent licensed advanced nursing professional providing medical treatment to patients at Clarks Summit State Hospital. The employee is responsible for a full range of healthcare services including, managing physical health problems and coordinating healthcare for psychiatric in-patients in accordance with CSSH policies, state & federal rules and regulations and the nursing standards of care; assessment of health status, diagnosis, development of plan of care and treatment, and evaluation of patient status. The range of healthcare services, includes planning, organizing, coordinating and directing optimal medical care and outcomes to ensure quality patient care to the adult and geriatric patients at CSSH. Clinical management is conducted under the general supervision of the collaborating medical physician(s). Administrative management is conducted under the general supervision of the Medical Director. The person in this position is an essential employee. |
Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor. 1.Functions independently to perform age-appropriate history and physical for complex acute, critical and chronically ill psychiatric in-patient patients. Identifies, measures, and monitors clinical and related outcomes to determine the effectiveness and appropriateness of the physical health plan. 2.Responsible for a comprehensive evaluation of assigned patients including but not limited to presenting illness, current and past health history, hospitalizations and treatment, social and developmental history. 3.Identify factors that may affect health such as family, trauma, genetics, substance use, spiritual influences and/or environmental factors. 4.Knowledge of the evidence-based practices and techniques used in the field of medicine and mental health, and the relationships between diagnostic findings and therapeutic methodologies. 5.Establish appropriate diagnostic and statistical coding in accordance with the current ICD coding guidelines. 6.Orders and interprets laboratory tests, procedures, imaging studies and consultations as indicated, to treat symptoms while improving functional health status under the direction of the collaborating supervisor(s). 7.Prescribes appropriate pharmacologic agents based on clinical indicators to treat symptoms and underlying causes of physical health disorders and assesses for any possible adverse effects. 8.Prescribes non-pharmacologic treatment or palliative modalities specific to patients’ needs such as, but not limited to, wound care, physical therapy, oxygen therapy, respiratory therapy, dietary plans, nutritional supplements/feedings, durable medical equipment, etc. 9.Monitors the patient’s treatment by making Unit rounds daily. 10.Collaborate with patients, family, interdisciplinary team members and the collaborating supervisor(s) in order to coordinate the patient’s care and on-going disposition planning. 11.Attends treatment team meetings to assist in the development of the ICTP. Identifies, monitors and measures clinical outcomes to determine the effectiveness of the physical health plan of care. 12.Attends and participates at CSP meetings. 13.Document on each patient at least monthly in a manner consistent with current documentation standards. 14.Facilitates the patient’s transition within and between health care settings, e.g. admitting, transferring, and discharging patients, and prepares all corresponding summaries and documents to support hand off communication. 15.Maintain a therapeutic relationship with patients, staff and families to promote positive clinical outcomes. 16.Participates in patient care as a member of the interdisciplinary team. 17.Complete billing forms as appropriate. 18.Consult with supervising physician(s) at least weekly and whenever the clinical complexity indicates. 19.Attend Joint Medical/Psychiatric Staff meetings. 20.Respond to medical and psychiatric emergencies, during which the CRNP will announce medical emergencies, direct verbal de-escalation, prescribe medication as appropriate and follow emergency procedures until the arrival of a staff physician. 21.Consultation with the collaborating physician and the Medical Director (or identified designee in the absence of the Medical Director) is required for a patient’s consent or refusal of surgery or other specialty care recommended interventions, end of life care plan, such as but not limited to, implementation of the Advanced Health Care Directive policy, hospice care, and/or Do Not Resuscitate guidelines. 22.Is available to attend all medical and psychiatric emergencies during the hours of 8:30am to 4:30PM, Monday through Friday. Participates in noon (1200-1300) coverage schedule on a rotating basis if needed under the supervision of a collaborating physician. 23.Coverage of other units during normal work hours with no increase in hourly rate, as assigned, in cases where the attending physician is off duty and according to the number of primary care physician staff. 24.Performs minor procedures including but not limited to simple repair of lacerations. 25.Performs related work as required. 26.CRNP will maintain requirements for licensure. |
