FSE Trainee This is a training level position in the Regional Forensic Psychiatric Center (RFPC). The trainee completes a fulltime training program, to include instruction and closely supervised on-the-job duties, to gain skills for providing care and treatment to mentally ill patients referred by the criminal justice system. Under the direction of a Forensic Security Employee Trainer and a Forensic Security Employee 2, the trainee will perform non-professional nursing care duties for patients with varying degrees of emotional and physical disabilities as well as be responsible for maintaining a therapeutic, secure environment and apply learned knowledge of policies and regulations required to perform the work. |

Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. FSE Trainee This position is supervised by the Forensic Registered Nurse (FRN) for purposes of clinical direction, performance evaluation and overall guidance of work performed on assigned unit(s) during the shift. The Forensic Security Employee Trainee will receive initial instruction and classroom training from an FSE Trainer and other designated professionals to establish competency. The Forensic Security Employee 2 provides lead work and oversight in carrying out daily routines to meet patient care needs and assignments for maintaining a safe, secure environment. The Forensic Registered Nurse provides safety, security and clinical oversight. The training program for the FSE Trainee will include instruction on all policies and procedures. Trainees will be expected to demonstrate understanding and application of all established policies and procedures in accordance with DHS, OMHSAS, State Hospital, RFPC rules and regulations in addition to compliance with maintaining the Patient’s Bill of Rights. Be aware of, familiar with, and follow all 7173, 7174, and 7178 policies. Completes all competencies required within the training program’s phase book with verification by the FSE Trainer, FSE2 and FRNS. Comply with OTJ training journal, complete entries daily, legibly, and in a manner that conveys new understanding of NHS policies and procedures with review of entries by FRNS weekly. Meetings are to be initiated by the FSE Trainee. Patient care duties learned through instruction, and application of on-the-job training, providing daily routine care for patients and ensuring a safe, secure environment for patients, staff and visitors to the facility. Demonstrate responsibility for continuous monitoring and supervision of patients, living areas and daily activities, during the evaluation and treatment process. Work closely with patients to accomplish day-to-day living activities and self-care skills such as bathing, oral hygiene, hair care, dressing and other personal care needs; provides assistance and guidance based on treatment plans; actively engages patients individual patient needs; incorporates daily routines to meet individual patient in a therapeutic manner, following clinical methods developed by treatment team. Acts as individual patient advocate to assigned patients by serving as a primary contact person, assisting with personal needs and belongings; shows respect to patients at all times and protects the right of confidentiality with patients, coworkers, and any other treatment partners. Perform simple nursing procedures, i.e., collects urine specimens, monitors and documents intakes and outputs, takes and records temperature, pulse, respiration, and blood pressure; measures the weight and height of the patients, monitors soaks and provides skin care and basic nursing care as instructed. Actively participate in treatment plan development, and implementation of interventions developed by the team; interact with patient to carry out daily routine of individualized plans using methods specified in treatment plans and encourage patient to participate with treatment program; maintain a safe, therapeutic environment; attend in-service training sessions to supplement knowledge and skills. Maintain respect and protect the rights of confidentiality of patients, coworkers, and other treatment partners. Complete daily, random environmental and face-to-face rounds of assigned areas including space used for patient programming to ensure integrity of the Forensic Center is not compromised; accountability rounds are completed for a minimum of 15 minutes to acertain patient wellbeing, i.e. is the patient walking, talking, breathing, safe, secure in the therapeutic environment. Encourage participation in programming, and document in accordance with applicable policies and procedures. Provide emotional support to patients offering encouragement through positive interaction in group and one-to- one settings. Observe, report and document legibly in the clinical record, incident report (SI-815), and other pertinent forms completing information relative to changes in patient’s mental and physical condition as noted through observation and interaction. Document in correlation with Individual Treatment Plan (ITP) and performance goals following Nursing/Forensic documentation policies and procedures. Attend and participate in shift report, taking responsibility for hand-off communication, reporting patient status, completed shift assignments, communicating clear, concise information to the FRN and the FSE2 lead worker before leaving duty and other times as necessary. Maintain a positive, respectful working environment and work collaboratively with coworkers, patients, all levels of management and others to make the best decisions to meet the needs of the patients and the facility. Recognize and respect professional boundaries to avoid conflict with patients, peers and leadership. Identify and address patients by name. Demonstrate performance of assigned work tasks, applying knowledge and skills learned through training and without the need for specific instruction beyond that normally provided by FRNS, FRN, and/or Forensic Security Employee 2. Recognize limitations of the trainee position in different phases and utilize the Forensic Security Employee Trainer, Forensic Security Employee 1, Forensic Security Employee 2, Forensic Registered Nurse, and Forensic Registered Nurse Supervisor to make collaborative decisions. Per policy, display State issued ID at all times. Security duties learned through training instruction and application of on-the-job training include: conduct daily security and Environment of Care rounds (scheduled and unscheduled) i.e. checks doors, windows and furnishings in accordance with applicable policies and procedures, report any deficiencies immediately; conducts random unannounced searches of patient living areas and secured areas i.e. gym, chapel, OT, etc. in accordance with applicable policies and procedures. Report any violation of policy and procedure to a supervisor immediately. Learn and demonstrate processes for assisting with admission, transfer and discharge of patients; escort patients to and from any area within the building or off grounds as assigned. Escort staff as needed. Learn and demonstrate use of escort and security procedures for patients escorted into the community for medical treatment or appointments. Screen visitors for various unauthorized materials including contraband; monitors visits, with eyes on patients and visitors to prevent security risks, observes activity to identify violation of policy i.e., pass contraband, etc. and ensures policy and procedure are followed at all times. Inspect mail and packages by trans frisking, logging, and inspecting all contents in the presence of the patient in accordance with RFPC procedures. Obtain operating knowledge of and appropriately utilize, all communication devices - including telephone, intercoms, two-way radios, entry systems and closed-circuit television including sally port functions; demonstrate learned knowledge and maintain responsibility for control of facility keys, reporting any loss immediately following policy. Implement security, fire, safety, infection control and emergency policy and procedures of the facility as appropriate; recognizes, documents, and determines disposition of contraband in accordance with policy. Communicate security issues to shift FSE 2/FRN, submit maintenance work order as needed. Conducts and documents internal security and safety inspections of the RFPC during the assigned shift. Utilize, Learn, practice and demonstrate appropriate use of techniques related to current crisis management interventions in accordance with state wide curricula to prevent or reduce escalating aggressive behavior in patients and others; maintain a safe, positive, therapeutic environment to foster respectful relationships with patients, coworkers and others. Report any threats of escape or rumors of unrest and participates in planning of prevention techniques. Makes rounds at change of shift and hourly to assess environmental and security risks. Reports any security or environmental issues identified to the FSE 2, FRN, and FRNS immediately. Make rounds and visually account for all patients in accordance with applicable policies and procedures. Actively prevents escape maintaining close observation of patients and being aware of their location and activity at all times. Report any security or environmental issues identified, to all coworkers especially, FSE 2, FRN, or FRNS immediately. Learn and apply implementation of Forensic code system for emergency responses and demonstrates appropriate use of code; respond and assist in emergency situations as indicated; performs CPR as necessary. Inspect all areas of the facility prior to use by Forensic patients. Participate in evacuation and disaster drills and adhere to emergency operations plan, demonstrates knowledge of the forensic evacuation procedure. Attend and complete all in-service training and educational programs as assigned to meet competencies i.e., CPR, crisis prevention/intervention techniques, AED use, etc.; completes self-learning packets as instructed. Report safety hazards and malfunctioning equipment to shift supervisor or other management staff; remove any broken furniture or unsafe items from living areas and notifies the immediate supervisor; maintains proper care, safety and use of hospital equipment. Participate in infection control, environment of care, and basic housekeeping functions within patient living areas, common areas, and staff designated areas; handles clean and soiled laundry in accordance with established procedures. Perform routine housekeeping duties to maintain environment in accordance with hospital policy. Dispose of trash and garbage properly. Requisition, inventory, and distribute supplies; maintain a clean, safe, and orderly supply area. Adheres to DHHS policy 7173 regarding time and attendance Perform other related duties and special projects as assigned. |
