Position Purpose:
Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. 

The Social Worker 2 is responsible for providing a full range of professional social services and support for resident, clients, resident representatives to plan, provide and coordinate needed social services.

Description of Duties:
Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor.

30% Participates as a member of the interdisciplinary care plan team by attending meetings and maintaining accurate and complete written documentation. Ensures all charted documentation is informative of the frequency and nature of social services provided to residents to meet BVH and SVH policy requirements and regulations of accredited agencies. Completes assigned sections of the resident assessment instrument on admission, quarterly, annually, and as needed basis. Integrates each resident’s psychosocial assessment with their person-centered plan of care.

25% Provides direct social work practice for residents with behavioral and psychosocial requirements. Arranges for or provides behavior management services to assist with the residents emotional or behavioral needs. Completes behavioral assessments and implements appropriate person- centered interventions for residents deemed in need of this specialized service. Offers consultation and education to other disciplines in developing behavioral management programs.

20% Assists in informing and reviewing residents’ rights, in notifying and screening regarding advanced directives for health care, grievance investigation and follow up, and in interviewing residents about voting status.

15% Counsels’ residents, their families, and others individually or in groups utilizing various treatment modalities such as supportive counseling, grief counseling, family therapy, crisis intervention, behavior modification, etc. to help them recognize the problems and to explore methods for adjusting, modifying, and improving personal and social functioning and behavior.

10% Coordinates discharge planning/transfers by addressing residents’ current needs including coordination with family/responsible party and supportive services, medical/psychiatric follow-up, coordination of transportation, and burial arrangements.

Maintain positive, constructive working relationships with co-workers; remain aware of the needs and sensitivities of others. Conduct job duties in compliance with established BVH and SVH policies, guidelines, and procedures.

Maintain and improve job skills and competencies by participating in and attending staff training programs in order to improve the quality of resident care.

Performs work in accordance with the DMVA general safety rules. Immediately reports all workplace injuries or accidents to the supervisor; and/or immediately corrects any safety hazards in the work area or reports same to the supervisor.

Attends all assigned QAPI training. Follows established policies and procedures in support of QAPI efforts. Supports QAPI efforts both verbally and non-verbally, including adjusting performance and practice in accordance with QAPI initiatives and findings.

Follows standard precautions. Uses required personal protective equipment such as gloves, masks, gowns, goggles, face shield, hearing protection, foot protection, etc.

Performs other related work duties as required.
