Position Purpose:
Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. 

This Social Work involves an assignment to our civil/ step-down units, which house male and female patients on court-ordered mental health treatment with judicial oversight. This employee functions as a member of a multidisciplinary treatment team and is involved in providing group treatment programs to the patients and maintaining the patients' community engagement with the county partners and families. Social Work responsibilities include daily discussion of discharge planning for the patients, the completion of initial and annual assessments, daily/ monthly required social work documentation in the patient's electronic health record (EHR), assisting with the development of a Treatment Plan with a multidisciplinary team, and the provision of appropriate referrals. The social worker serves as a liaison/therapist/resource to families and community agencies. Discharge planning is done with recognition of the special needs due to severe and pervasive mental illness, legal charges, recurring medical conditions, and physical limitations. The Social Worker must observe all reporting requirements of the hospital’s Social Work Department and accrediting agencies, such as Medicare and Medical Assistance.

Description of Duties:

-Develops information regarding patients assigned to this social worker, in compliance with departmental standards, for the purpose of completing an assessment, developing a treatment plan, effectuating treatment, and planning for discharge and aftercare.
-Conduct initial/annual interviews with family, friends, or significant others; gather material submitted by BSU, inpatient units, and others, as well as conduct personal or telephone interviews for the purpose of developing a thorough knowledge of the assigned patient and their living situation. This information is then organized into the patient's Electronic Health Record (EHR) while maintaining departmental standards.
-The incumbent in this position will ensure that all required forms, including, but not limited to, information release, patients’ rights, personal possessions, Medicare, Medical Assistance, or Hospital or Departmental Standards, are completed within identified time frames.
-Serves as a member of the unit’s multidisciplinary team to contribute social work data and insights for use in treatment discharge planning.
- Drive patients to interview and discharge placements when required as part of the patient’s discharge planning process.
- Provide coverage to peers when directed by Social Work Leadership.
- Maintain appropriate hospital training needed to support the client in case of a crisis.
-Attends meetings as scheduled.
-Ensures that the charts of all patients scheduled for treatment planning/review have social work documentation, according to standards established or ascribed to by the Department.
-Establishes and maintains effective task-oriented relationships with treatment team staff.
-Provides professional social services to clinical staff to ensure compliance with Departmental and Hospital Standards and the coordinated provision of quality social work services.
-Meets with representatives of the other clinical staff as needed to assist in evaluating problems interfering with patient recovery and utilizes social work knowledge and experience in identifying and developing approaches or techniques to solve these problems.
-Provides therapeutic services to patients based on a thorough knowledge of background information; a clinical assessment of needs/strengths; in accordance with treatment plans formulated by the treatment team, and in accordance with accepted treatment modalities.
-Develops a formal or informal psychosocial assessment, which includes a profile of the patient, family, and environment; relative strengths/weaknesses of each, and the manner in which these systems interrelate.
-Initial intake assessment is completed in accordance with established Departmental Standards for recently admitted patients. Subsequent assessments are completed when required by the treatment team to plan treatment or evaluate the course of treatment, and in compliance with Departmental Standards.
-Revises social work treatment strategies based on assessment, which are best suited to the individual and his/her situation. These treatment strategies should be appropriate to the overall treatment goals established by the treatment team.

Decision Making:

Identify the problems or issues that can be resolved at the level of this position, versus those that must be referred to the supervisor. -Develops skills through readings, participation in in-service training programs, and discussion of clinical issues involved with treatment.
-Serves as the primary member of the multidisciplinary team in the assessment of the client system about available resources.
-Encourages patient participation in the identification and selection of needed resources.
-Provides sufficient information to the selected human service providers to effectuate needed referrals.
-Selectively acts on the patient’s behalf by appearing with or for the client at an agency, by writing letters and making phone calls to ensure the client will obtain needed resources.
-Documents the outcome of referrals.
-Maintains a current knowledge of community resources.
-Develops community resources, when unavailable, which will benefit the patient.
