Position Purpose: Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. This is professional and administrative work assisting with the risk management (RM) and quality improvement (QI) program operations for the facility. The employee in this position organizes and supports the efforts of an integrated, interdisciplinary, professional staff by managing, analyzing, and presenting information related to all quality improvement functions at the facility. In collaboration with the QA/RM Coordinator work will involve developing and implementing RM and QI operating structures, evaluation studies and reports to monitor practices and service delivery. The employee collaborates with clinical managers, facility administrative staff, and department officials to establish monitoring of QI goals, objectives, standards and criteria to evaluate the quality and safety of individual care. |

Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. The employee in this position plans, develops, coordinates, monitors, and evaluates an integrated facility-wide information management and analysis program. The employee in this position, in collaboration with administrative staff, develops monitoring and evaluative benchmarks and standards for the facility to meet QI goals and objectives. The employee in this position develops reports about all RM and QI activities to ensure that risk to individuals is minimized, quality of life is maximized, and the facility is in compliance with departmental objectives. The employee in this position provides technical assistance and referral information about information management, data analysis, and improvement strategies. The employee in this position is responsible for the collection, organization, analysis and presentation of data for the facility. The employee in this position is proficient in using a variety of diagrams, flowcharts, histograms, control charts, run charts, Pareto charts, Force Field Analysis, Failure Mode Effect Analysis, Comparative Analysis, Time Analysis, etc. to monitor, analyze and disseminate data. The employee in this position is responsible for the development and maintenance of facility databases, monthly event calendars, dashboards, and other weekly, monthly and quarterly reports as required. Demonstrates knowledge of Enterprise Incident Management(EIM)System. The employee in this position provides the technical assistance to facility administrative and management staff regarding electronic data operations (e.g.: QI Macros, ACCESS, EXCELL, Powerpoint). The employee in this position provides the technical assistance to facility administrative and management staff regarding data organization tools and analysis (e.g.: diagrams, flowcharts, histograms, control charts, run charts, Pareto charts, Force Field Analysis, Failure Mode Effect Analysis, Comparative Analysis, and Time Analysis). The employee in this position provides the technical assistance to facility administrative and management staff regarding EIM/HCSIS operations (e.g.: Data Warehouse, ESS, etc). The employee in this position assists in all incident management and risk management activities as directed and as needed. The employee in this position, in collaboration with the applicable management and administrative staff, implements IM, RM and QI activities, in medical staff audits, pharmacy and therapeutic services, risk management, infection control, restraint review, medical records, safety, Life and individuals’ rights. The employee in this position initiates studies to analyze trends or patterns of unusual incidents, habilitative care, treatment interventions, practitioner performance, non-clinical support activities, community placements and other living area transitions to identify opportunities for improvement. The employee in this position, in collaboration with administrative staff, develop action plans, annual plans, data collection systems, to assist with and manage improvement goals in both clinical and habilitative services. The employee in this position is a member of committees and participates at meetings of appropriate professional clinical and non-clinical staff and committees (e.g.: Executive Staff, Risk Management Committee, Life Committee, Safety Committee, Fall Committee, Skin Management Committee, PICA Committee, Certified Investigations, CI Peer Reviews, as well as daily RM meetings, and other meetings as designated by the facility). The employee in this position provides the technical assistance to other facility staff regarding the development of electronic presentations and use of the equipment. The employee in this position may participate in the performance of other Risk Management functions consistent with operational or organizational requirements. The employee in this position demonstrates proficient facilitation skills in assisting teams and work groups to develop and implement strategic program plans, annual QM plans, and other operational or organizational improvement activities. The employee in this position is required to be a Certified Investigator and conduct Certified Investigations as assigned. The employee must maintain certified investigator status. The employee in this position adheres to the expectation to conduct certified investigations objectively, efficiently and thoroughly using standard protocols to identify, collect and analyze evidence available to an investigation. The employee in this position ensures that all incidents that require an investigation receive a systematic investigation that meets established standards. The employee in this position demonstrates knowledge of the Incident Management Bulletin, ICF/IID regulations and Incident Management policies related to abuse prevention. The employee in this position participates in state wide focus area meetings, web collaborations, trainings and committee meetings as well as workgroups as directed and as needed. The employee in this position participates in staff training related to QI, IM and RM. The employee in this position provides training related to Quality Improvement, Incident Management and Risk Management functions, tasks and goals. The employee in this position performs other related duties as required. |
