Position Purpose: Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. This position reports directly to the Facility Director. This position is responsible for complex professional work planning, directing and monitoring multiple departments at a licensed ICF with the capacity of 401 beds. Departments under the QARMD include the quality improvement, risk management, program operations, medical records and staff development for the facility. The employee in this position plans, organizes, and directs the work of an integrated, interdisciplinary, professional staff that monitors and evaluates the continuum of services provided by the facility with the objective of continuous quality improvement. Work includes interpreting federal and state requirements and developing and implementing QI operating structures, policies, procedures, evaluation studies and reports to monitor practices and practitioner performance in delivering clinical services and providing non-clinical support. The employee reports directly to the Facility Director and collaborates with division directors, managers, facility staff, and department officials to establish QI goals, objectives, standards and criteria to identify quality indicators and measure the quality, appropriateness, and individual satisfaction in comparison with national, state and facility specific care standards. Considerable initiative and independent judgement are required in coordinating programs and decision making. |

Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor. The employee in this position plans, develops, directs, and evaluates an integrated facility-wide interdisciplinary clinical and non-clinical complement under the QARMD including Quality Assurance and Risk Management, Staff Development and Medical Records. Collaboration with Executive staff and management staff to develop benchmarks and standards for the facility to meet departmental goals. Assists Facility Director, Clinical Director and Program Director in the development of long-range policies, plans and goals for the Center and the residents. Provides technical assistance and referral information about habilitative services oversight. The employee in this position, in collaboration with administrative staff, develops operational plans, policies, procedures, and timetables for Quality Assurance and Risk Management activities to be implemented, monitored, assessed and evaluated. Oversees and participates in DOH surveys and Plan of Correction to ensure compliance. Organizes and consolidates ongoing facility-wide service delivery monitoring to maximize the quality of life of those being served and ensure the quality care. The employee in this position provides direction for the risk management program, directs the review of reports about incidents, medication, and third party liability and the investigation of problems, complaints and inquiries to recommend appropriate corrective action. The employee in this position conducts need assessments and develops goals and objectives for the facility's annual plan and budget requests to insure cost effective improvements in individual care. The employee in this position consults with Facility Director, Division Directors, management, department heads and other clinical staff for clinical credential requirements and non-clinical practice issues affecting health care, treatment, habilitation and safety. The employee in this position, in collaboration with the applicable management and administrative staff, monitors QI activities, in medical staff audits, pharmacy and therapeutic services, risk management, infection control, restraint review, medical records, safety, and individuals’ rights. The employee in this position develops and administers systems to evaluate the relative efficiency and effectiveness of various patterns and modes of service delivery for treatment programs. The employee in this position initiates studies to analyze trends or patterns of unusual incidents, habilitative care, treatment interventions, practitioner performance, non-clinical support activities, community placements and other living area transitions to identify opportunities for improvement. The employee in this position, in collaboration with appropriate Executive staff and administrative staff, evaluates consumer satisfaction information from the IM4Q and LIFE monitoring to ensure that facility services are responsive to individual wants, needs and desires. The employee in this position Chairs or participates at meetings of appropriate professional clinical and non-clinical staff and committees including Board of Trustees, HRC, Executive Staff and Risk Management, others as assigned. The employee in this position monitors, reviews and completes evaluations on the performance of subordinates work consistent with operational or organizational requirements. Informal reviews and annual EPR completed for subordinate staff. The employee in this position, in collaboration with the Executive team and administrative staff, directs facility or departmental special projects, and monitors outcomes of same. Familiar with and meets all state, local and federal standards which govern the operations of the Center, specifically all standards associated with ICF/IID license. The employee in this position performs other related duties as required. |
