Position Purpose: Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. This position is responsible for delivering medical and psychiatric nursing services to adjudicated delinquent residents who are court committed to the Youth Development Center/Youth Forestry Camp (YDC/YFC) system. This position will provide care under the consultation of a medical physician/CRNP, psychiatrist, optometrist and dentist. These responsibilities are performed in accordance with the Department of Human Services (DHS), Office of Children Youth and Families (OCYF), BJJS and agency policies. This position reports directly to the Registered Nurse Supervisor. |
Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor. Follows physician/CRNP orders for medications and treatments by checking and signing the physician/CRNP orders and transferring the same information onto the residents’ medication record in order to ensure sound method of distributing medications and treatments. Prepares and distributes medications to residents and maintains proper storage of medications by following established procedures in order to ensure a proper audit of medications. Maintains medical logs and individual medical records on residents by recording all medications, treatments and evaluations in a clear and accurate manner in order to have documented evidence of care provided. Conveys essential medical needs of residents to the oncoming shift. Conveys essential medical needs of residents to cottage/dorm staff for continuity of medical care. Provides contracted medical providers medical information for the continued care of residents. Prepares residents medical reports for court documents/reports and/or other reports as indicated. Attends and participates in multidisciplinary team meetings, MCPC’s, and other interdisciplinary team meetings as indicated. Maintains accurate medical documentation in resident files and the medical log. Assures that all residents have a Health & Safety screen completed within the time frame designated by 3800 regulations and local policy. Utilizes JJACS as indicated. Maintains PREA standards and utilizes associated methods for reporting as indicated. Makes the appropriate referrals for the ongoing medical care of residents. Reports any unusual medical and behavioral symptoms of residents to the Designated Health Care Authority immediately. Reports any behaviors that are seen to be of potential harm to residents to the Designated Health Care Authority. Attends training as related to the performance of job duties. Administers immunizations to residents as per the recommendations of the PA Department of Health. Enters resident immunization records and/or administration of immunizations in the PA Statewide Immunization Information System (SIIS). Administers immunizations to staff as necessary. Serves as a liaison between physicians and the resident. Attends Psych/Medical meetings as indicated for residents maintained on psychoactive medication. Schedules resident appointments with Primary Care Provider. Maintains schedule of yearly physical examinations for residents. Maintains schedule of routine dental examination for residents. Maintains schedule of immunizations for residents. Assures that all medical needs of residents are addressed. Orders medication for residents as ordered by the attending physician/CRNP. Monitors the usage of medications and reports effects to the attending physician/CRNP. Checks all medication and medical supplies for expiration dates and reports to the Designated Health Care Authority or designee. Arranges for transport of residents to the medical area for care. Schedules and coordinates off-grounds medical appointments and transportation for residents. Provides emergency care to residents by evaluating the injury or sickness to determine treatment necessary or referral to supervisor, doctor/CRNP or local medical facilities (e.g. hospital emergency room) in order to provide adequate assessment and/or treatment. Provides emergency care to employees until appropriate medical care can be obtained. Acts as a liaison between medical and direct care staff. Provides CPR and First Aid Certification, HIV training and health education. Adheres to all federal, state and agency regulations, policies and procedures governing confidentiality, safety and security. Applies the principles of Balanced and Restorative Justice (BARJ) and consider these principles during the development and approval of case management plans and other work situations as required. Attends and participates in staff meetings, supervisory conferences and employee training programs, including but not limited to Crisis Intervention Suicide Prevention, Safe Crisis Management, CPR/AED/First Aid and Cultural Diversity in order to develop and maintain position skills. Works overtime as required by operational necessities and emergencies. Must be able to work mandatory overtime in compliance with overtime provisions of the collective bargaining agreement, with little or no advance notice, whether on or off duty at the time of notification. Travels to satellite facilities as necessary to provide nursing services. Performs related duties as required. Prepares residents medical reports for court documents/reports and/or other reports as indicated. |

Determine appropriate plan and delivery of care for residents’ routine and/or emergent medical needs in compliance with approved policy, procedures and standing orders. Maintain effective communication with intra-disciplinary team members, inter-disciplinary team members, and outside providers as necessary to ensure efficient health care of residents. Report any of the following situations to the Nurse Supervisor immediately: Illness/injury to a resident or staff member which requires off ground medical attention including hospitalizations, any suicide or suicidal gestures, any incident that could result in serious allegations of illegal or unprofessional behavior on the part of facility staff members, any alleged criminal activity occurring at the facility, any incidents of alleged resident abuse, any injury/incident involving a visitor or volunteer, and any serious communicable disease. |
