Position Purpose:
Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. 

The employee occupying this position is responsible and accountable for providing basic health care needs for assigned residents in support of the overall goal to provide the highest quality of care and services to our residents in order to assist them to achieve their full potential at the Center and in the community.

Description of Duties:
Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor.

Planned working time is approximate and can change from day to day based on operational needs.

70% of working time is in direct resident care:
• Provides basic care to residents by meeting their personal hygiene, physical and emotional needs in order to maintain a healthy, clean and safe environment while meeting basic licensing requirements.
• Participates in rehabilitation/restorative programs for residents by assisting professional staff to help residents achieve their highest level of potential for independence and self-fulfillment.
• Utilizes therapeutic communication skills and crisis prevention techniques in interaction with residents. Acts as resident advocate.
• Contributes to each resident's Plan of Care by attending Interdisciplinary Team meetings to discuss treatment plans and develop individualized goals and methods to approach these goals for rehabilitation/maintenance of residents. Performs nursing duties per procedure to provide a clean, orderly, and sanitary environment.
• Teaches residents to make beds and care for personal possessions within their limitations and performs duties for those who are unable. Observes and assists at mealtime and with nourishments, teaching proper eating habits and promoting intake as necessary. Utilizes infection control techniques.
• Communicates changes in status of both resident and physical environment by verbal/written notification to immediate supervisor or other appropriate designated person in order to provide consistency of care

20% of working time:
• Assists in transportation of residents by accompanying residents to and from health provider facilities and by escorting residents in order to ensure attendance at appointments. Assists and encourages resident attendance and participation in activities and other therapeutic rehabilitation programs to improve their awareness of self and others; their functioning and social interaction.

5% of working time:
• Maintains and improves necessary job skills by participating in and attending staff training programs in order to improve the quality of care of the resident.

5% of working time:
• Assist with routine procedures pertaining to admissions, transfers and discharges of residents.

Makes resident care decisions based on training and in consultation with professional nursing staff, i.e. LPN, LPN Supv, RN, RN Supv, Nurse Manager and or Director of Nursing.
