Position Purpose: Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. This position is responsible for delivering medical and psychiatric nursing services to facility residents who are court committed to the Youth Development Center/Youth Forestry Camp (YDC/YFC) system. This position will provide care under the consultation of a medical Physician/PA-C/CRNP, Psychiatrist, Dentist, and Optometrist. These responsibilities are performed in accordance with the Department of Human Services (DHS), Office of Children Youth and Families (OCYF), Bureau of Juvenile Justice Services (BJJS) and agency policies and procedures. This position reports directly to the Registered Nurse Supervisor. |

Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor. Follow Physician/PA-C/CRNP orders for medication and treatment by checking, signing, and transferring the same information onto the residents’ medication record to ensure a sound method of distributing medications and treatments. Order medication for residents as ordered by the Physician/PA-C/CRNP. Prepare and administer medications to residents and maintain proper storage of medications by following established procedures to ensure a proper audit of medications. Check medication and medical supplies for expiration dates and report to the Chain of Command or designee. Assure that all residents have a Health & Safety Screen completed within the time frame designated by 3800 regulations and local policy. Ensure all resident care, encounters, and evaluations are documented accordingly. Schedule acute and chronic care appointments with the facility Physician/PA-C/CRNP. Maintain schedules of routine physical examinations, dental examinations, and optometry examinations, as well as psychiatric evaluations (as necessary) for residents. Assure that all medical needs of residents are addressed in a timely manner. Effectively communicate the essential medical needs of residents to cottage/dorm staff for continuity of medical care. Report any unusual medical, behavioral, or potential harmful behavior of residents, as well as medication refusals, to the appropriate facility and/or specialty provider, chain of command, and treatment team immediately. Make the appropriate referrals for the ongoing medical care of residents. Provide contracted medical providers’ pertinent medical records for the continuity of resident care. Ensure lab results, radiological results, Emergency Department paperwork, off-grounds appointment referral paperwork and clinical notes, or medical records are obtained in a timely manner. Review, verify, sign and date resident medical bills/invoices to confirm services rendered. Maintain thorough and organized medical records on residents. Convey essential medical information related to residents and/or department to the oncoming shift. Complete departmental duties, medical logs and/or shift reports accordingly. Provide HIV/Heb B/Hep C and ongoing health and wellness education to residents. Utilize JJACS as indicated. Maintain Childline and PREA standards and utilize associated methods for reporting as indicated. Prepare residents’ medical reports for court documents/reports and/or other reports as required. Attend and participate in MCPC’s, Youth Team Meetings (YTM), Discharge, and other multidisciplinary team meetings as indicated. Act as a liaison between medical and program staff. Serve as a liaison between medical/psychiatric providers and residents. Attend psychiatric meetings as indicated for residents prescribed medication by the Psychiatrist. Administer immunizations to residents as per the recommendations of the PA Department of Health. Enter resident immunization records and/or administration of immunizations in the Pennsylvania Immunization Electronic Registry System (PIERS). Maintain schedules of immunizations for residents. Arrange for transport of residents to the medical area for care. Schedule off-grounds medical appointments for residents and ensure information is relayed to program staff. Assist in coordinating off-grounds medical appointment transportation of residents as indicated. Attend resident off-ground appointments with program staff as assigned by chain of command. Provide emergency care to employees until appropriate medical care can be obtained. Administer immunizations to staff as necessary. Adhere to all federal, state and agency regulations, policies and procedures governing confidentiality, safety and security. Apply the principles of Balanced and Restorative Justice (BARJ) and consider these principles during the development and approval of case management plans and other work situations as required. Attend and participate in staff meetings and supervisory conferences. Attend, participate, and pass mandatory employee training programs, including Safe Crisis Management (SCM) to develop and maintain position skills. Work overtime as required by operational necessities and emergencies. Must be able to work mandatory overtime in compliance with overtime provisions of the collective bargaining agreement, with little or no advance notice, whether on or off duty at the time of notification. Travel to satellite facilities as necessary to provide nursing services. Maintain standards of attendance and follow call-off policies and procedures. Maintain cleanliness of department and workspaces. Review and co-sign applicable documents completed by a Licensed Practical Nurse (LPN) as indicated. Perform related duties as required. |
Decision Making: Describe the types of decisions made by the incumbent of this position and the types of decisions referred to others. Example: In response to a customer inquiry, this work involves researching the status of an activity and preparing a formal response for the supervisor’s signature. Determine appropriate plan and delivery of care for residents’ routine and/or emergent medical needs in compliance with approved policy, procedures, plan of care, and standing orders. Review lab results, radiological results, and off-grounds appointment referral or Emergency Department paperwork and clinical notes to their entirety and notify the appropriate medical and/or psychiatric provider of abnormalities and/or recommendations and outcome. Maintain effective communication with departmental team members, multidisciplinary team members, and outside medical providers to ensure efficient health care of residents. Report any of the following situations to the Nurse Supervisor and/or Nurse Manager immediately: Illness/injury to a resident or staff member which requires off ground medical attention including hospitalizations, any suicide or suicidal gestures, any incident that could result in serious allegations of illegal or unprofessional behavior on the part of facility staff members, any alleged criminal activity occurring at the facility, any incidents of alleged resident abuse, any injury/incident involving a visitor or volunteer, and any serious communicable disease. |
