Position Purpose: Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. This position has primary responsibilities for the coordination and implementation of transitional services at South Mountain Secure Treatment Unit (SMSTU). More specifically, the incumbent ensures that the facility successfully transitions youth back into their community through leading an active process of discharge planning, establishing aftercare a network of community-based resources and conducting follow-up after youth leave a Youth Development Center/Youth Forestry Camp (YDC/YFC) system facility. This position's work will also include interpreting federal, state and Department of Human Services legislation, regulations, policies and procedures. The incumbent will provide consultative services to other YDC/YFC employees, youth, family members, and/or community agencies. Work also includes providing advice, guidance and technical assistance for compliance with regulatory standards and for meeting the YDC/YFC system’s goals and objectives related to the ongoing development of a comprehensive system of transitioning youth back into the community. |
Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor. 1. Actively participate in and support the program’s multidisciplinary process and the overall Master Case Planning System. This includes assuming responsibility for the aftercare/discharge portions of the related documents. 2. With the understanding that aftercare planning begins at referral, initiate and maintain a focus on aftercare and transitional planning with the resident’s parent(s)/guardian(s), family, probation officer and facility staff 3. Through the use of presumptive eligibility and/or working with a resident’s private insurance carrier, ensure that youth have access to and funding for medical and behavioral health services upon release. 4. Identify the needs of the youth, their family and/or social support system. This process shall include, but not be limited to, the use of normed/validated risk and need assessments. 5. Facilitate the development and implementation of competency based groups related to the transitional issues of youth in the Implementation Phase of their program. 6. Facilitate the development and implementation of treatment groups related to the transitional issues of youth in the Transitional Phase of their program. 7. Support the youth and facility through participating in court hearings as determined necessary and appropriate. 8. Support Performance Based Standards through collecting and maintaining relevant data and participating in related facility-based initiatives, which includes administering the PbS Youth Exit Interview two weeks prior to the anticipated discharge hearing. 9. Coordinate with outside agencies and establish a resource network that supports youth leaving the YDC/YFC system. 10. Primary responsibility for the facility’s overall aftercare/discharge planning process, to include but not limited to, establishing follow up appointment dates and times for community-based resources. 11. Support programs and youth through initiating and completing benefit paperwork in cases of resident inpatient hospitalization. 12. Facilitate the identification, connection to and funding of appropriate academic, career and vocational programs for youth upon release. 13. Create and maintain a “Discharge Portfolio” for youth leaving the facility. 14. Facilitate the process by which residents receive copies of relevant vital documents, including birth certificates and social security cards. 15. Conduct follow up related to a resident to ascertain if they’re participating in their transitional/aftercare plan or if additional supports need to be brokered. 16. Attend and participate in staff meetings, supervisory conferences and employee training programs, including but not limited to Crisis Intervention/Suicide Prevention, Safe Crisis Management, CPR/First Aid, Cultural Diversity and Certificate Programs, including but not limited to: general counseling, substance abuse, mental health and/or developmental disabilities, in order to develop and maintain position skills. 17. Conduct a transitional orientation with the resident, individually or as a group. 18. Ensure that a Youth Level of Service Inventory (YLS) is completed within the required timeframes specified by the PA JJSES YLS Subcommittee. 19. Provide a copy of the YLS report to the Youth Development Counselor (YDC), YDC Supervisor, Drug and Alcohol Treatment Specialists and Psychological Services Associates. 20. Ensure copies of identified documents are placed in the resident’s main file. 21. Develop a draft of the initial “Transition and Reintegration” Plan and provide the YDC with a copy prior to the Master Case Planning Conference (MCPC). 22. Participate in the Master Case Planning Conference (MCPC) and complete required documents as outlined in the MCPS and as assigned. 23. While at the MCPC, ensure that the initial transition and reintegration plans are accurate. If not, make the necessary revisions and email a revised copy to the YDC within 3 business days of the MCPC. 24. Participate in monthly MDT meetings by completing the “Transitional Services/Reintegration Summary” section of the Monthly Report or the “Aftercare Planning” section of the Court Report and Discharge Summary. 25. Obtain standard releases of information for transition service providers (Parent/guardian, School, psychiatrist, JPO, State Court Unit, MH/MR, D&A, etc.) and place it in the main file. 26. Develop discharge plans for youth in the event of an unexpected discharge. 27. Maintain a safe, secure and therapeutic environment. 28. Successfully complete mandatory annual training, Juvenile Justice Academy and other trainings as assigned. 29. Provide trainings to employees, youth and individuals outside of the YDC/YFC system as determined appropriate. 30. Represent the Bureau of Juvenile Justice Services at meetings and committees as determined appropriate. 31. Adhere to federal, state and agency regulations, policy and procedures governing confidentiality. 32. Communicate the status of both the residents and their physical environment through verbal and written notification according to established procedures in order to provide consistency of care. 33. Maintain order and discipline by intervening with residents through active supervision, responding to emergencies and employing the least restrictive intervention technique in order to prevent escapes, control inappropriate behavior and maintain a safe and secure environment. 34. Escort and supervise residents to and from various activities, clinics, appointments and visits on and off-grounds as determined appropriate 35. Work overtime as required by operational necessities and emergencies. 36. Identifies, interprets and implements best practice standards. 37. Participates in court hearings as necessary. 38. Perform related duties as required. |
