Position Purpose: Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. The employee in this position is responsible and accountable for attending and completing offsite training in order to be certified as a Nurse Aide. Nurse Aides are responsible for providing basic health care needs for assigned residents in support of the overall goal to provide the highest quality of care and services to our residents in order to assist them in achieving their full potential at the veterans home and in the community. |
Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor. Within 3 months attend formal classroom training sessions and receive on-the-job training assignments in order to develop knowledge, skills, abilities, and attitudes necessary to provide non-professional rehabilitative nursing care and treatment for residents. Classroom training entails activities such as reading assignments, listening to lectures, watching films, observing demonstrations, and role playing. Employees receive clinical site training in hands-on non-professional direct care methods, techniques, and procedures. As knowledge and skills are acquired, further developmental assignments are given in actual work situations. Maintain and improve job skills by participating in and attending staff training programs in order to improve the quality of resident care. Make rounds with personnel at beginning and end of shift. Maintain a safe environment in resident rooms by removing and addressing hazards such as electrical cords, personal effects, and other obstacles. Assist resident with room tidiness, cleanliness, and organization. Assist residents with opening, reading, and disposing of mail as applicable. Utilize infection control techniques during all duties. Make and change linens for unoccupied beds. Distribute clean clothes and clean linens. Answer call bells and assist with basic needs such as requests for snacks/water and TV assistance. Inform nurse of request for clinical care needs such as toileting, positioning, medication management. Act as a resident advocate. Transport tray carts to and from serving areas. Distribute and collect meal trays, and nourishments. Promote intake as necessary. Distribute ice water/thickened liquids and between-meal nourishments. Take and record temperatures on pantry refrigerators. Document on residents as assigned in writing and thru use of electronic documentation system. May assist with feeding residents if: (i) The trainee has successfully completed a state-approved training course that meets the requirements of §483.160 before feeding residents; and (ii) The use of feeding assistants is consistent with state law. The trainee must only feed under the supervision of a Registered Nurse (RN) or Licensed Practical Nurse (LPN). In an emergency, the trainee must call a supervisory nurse for help. The trainee is only permitted to assist with feeding of residents who have no complicated feeding problems. Contribute to residents’ Plans of Care by attending Interdisciplinary Team meetings to discuss treatment plans and develop individualized goals and methods to approach these goals for rehabilitation/maintenance of residents. Assist residents during scheduled activities and programs as directed. Transport residents by wheelchair, Geri chair, stretcher, etc. in accordance with Department policy. Transport residents and participate in such program activities as: social parties, dances, dinners, bus/van rides, table games, field trips, clubs, movies, bingo, shopping, bowling, crafts, games, fishing, music, exercise, picnics, gardening, and leisure time spectator activities. Clean medical equipment such as Nebulizers, IV Pumps, Glucometers, Stethoscopes, Feeding Pumps, wheelchairs, Geri chairs, tray chairs, etc. as needed. May monitor the smoke room to ensure safety of residents. May monitor the front door/other exit doors for resident safety. May monitor the residents outdoors, onsite for safety. Empty waste and dirty linen containers to proper areas and dispose into trash dumpsters. Collect and dispose infectious waste into proper storage areas. Empty urinals, bedpans. Keep utility rooms cleaned, neat, and organized. Restock resident supplies such as toiletries, urinals, toothbrushes, basins, etc. Pick up and/or deliver supplies for the nursing unit. Introduce residents to roommates, other residents, and to staff; record all personal belongings brought in or acquired later; prepare clothes for marking; and familiarize residents with the physical layout of the facility. Function as the greeter for new admissions. Pack/unpack resident belongings upon admission/discharge/room change. Review empty beds for room readiness prior to new admissions/room transfers. Maintain positive, constructive working relationships with co-workers; aware of the needs and sensitivities of others. Conduct job duties in compliance with established policies and procedures. Report unusual behavior, safety concerns, defective equipment, or unsafe housekeeping conditions immediately to the nursing supervisor on duty. Perform work in accordance with the DMVA general safety rules and safety rules for this position. Immediately report all workplace injuries or incidents to the supervisor; and/or immediately correct any safety hazards in the work area or reports same to supervisor. Support the facility QAPI efforts by attending all assigned QAPI training; following established policies and procedures in support of QAPI efforts; participate in PIP teams as assigned; sharing ideas for improvement in the organization; and supporting QAPI efforts both verbally and non-verbally, including adjusting performance and practice in accordance with QAPI initiatives and findings. Follow universal precautions. Use required personal protective equipment such as gloves, masks, gowns, goggles, face shield, etc. Perform other related duties as required. ** Complete course work, clinical training, and testing to become a state registered Nurse Aide within 6 months of hire.** |
