Position Purpose: Describe the primary purpose of this position and how it contributes to the organization’s objectives. The employee occupying this position is responsible for the development, implementation, monitoring and supervision of the Wound Care program and the Restorative Nursing Program, and will function as the back up support for the Infection Control Nurse for the facility. This employee is the liaison between therapy and nursing and promotes a restorative nursing philosophy among the facility departments |

Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Does assessments, makes recommendations regarding prevention of skin breakdown and maintaining healthy skin integrity. Provides treatment to wounds and evaluates effectiveness of treatment regime. Assures compliance with all regulatory requirements related to wound and skin care. Acts as a liaison between regulatory agencies. Is responsible for initiating infection control measures under the direction of the Infection Control Committee. Is responsible for meeting infection control standards of regulatory agencies and keeping the Director of Nursing appraised of reportable diseases. Coordinates the Employee Health Program at the Gino J Merli Veterans’ Center. Maintains logs and records of each transaction. Reports directly to the Clinical Service Manager/DMVA and/or Assistant Clinical Service Manager/DMVA with regards to the program and any other duties attributed to this program. Other related duties as required including conducting monthly wound care meetings. Performs work in accordance with the DMVA general safety rules and safety rules for this position. Immediately reports all workplace injuries or incidents to the supervisor; and/or immediately corrects any safety hazards in the work area or reports same to supervisor. Supports the facility QAPI efforts by attending all assigned QAPI training; following established policies and procedures in support of QAPI efforts; participate in PIP teams as assigned; sharing ideas for improvement in the organization; and supporting QAPI efforts both verbally and non-verbally, including adjusting performance and practice in accordance with API initiatives and findings. Assists with the development of policies and protocols for skin care, prevention of breakdown and treatment of various state wounds. Develops care plans and interventions for high-risk residents and documents as necessary. Provides staff education regarding prevention of skin breakdown. Oversees and monitors the skin care prevention program to make sure staff are following through with active measures to prevent skin breakdown and that all residents that are at risk for skin breakdown are identified. Provides a weekly wound report to the Medical Director, Director of Nursing, and Quality Assurance Coordinator. Monitors on-going documentation and assessment of wounds by staff nurses. Conducts audits of documentation and reports to the Quality Assurance Committee. Is responsible for all data input into the computer system. Performs all other related duties Obtain and maintain CPR certification. |
