Position Purpose:
Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. 

This position is responsible for providing professional oversight and consultation for the clinical components of all long-term care veterans’ homes within the Bureau of Veterans Homes (BVH) under the State Veterans Homes (SVH) system. This position’s primary focus is quality measures (QM), minimum data set (MDS), and MatrixCare informatics and their integration with the Agency’s Quality Assurance and Performance Improvement (QAPI) program. Incumbent serves as the BVH QAPI program’s primary interface with SVH’s nursing leadership. Incumbent reports directly to the QAPI Administrator, performs day to day functions at various assigned work locations and conducts onsite visits to the SVH’s.

25% Informatics: Serves as the BVH POC for clinically oriented, technical, and training matters regarding MatrixCare, CASPER and the MDS. Assists QAPI Administrator and QAPI Program Analyst with quality measure oriented data retrieval and analysis. Provides relevant quality measures training to SVH quality and nursing staff in concert with BVH QAPI program goals.

15% Proficiency: Responsible for developing, administering, and reviewing a certification process that reflects current standards of competent performance in clinical assessment and care planning for the SVHs using the RAI/MDS process. Provides oversite and direction of the SVH Registered Nurse Assessment Coordinators (RNAC) to ensure timely and accurate completion of all OBRA and Medicare assessments to comply with federal regulations. Monitors the overall process at the SVHs for tracking of MDS assessments including completion, submission, and correction as needed.

15% Regulatory: Interprets regulations, laws, standards of care, policies, and procedures as they are related to long term and personal care, including nursing and quality services as well as ancillary services. Ensures consistency and standardization in the implementation of policies and procedures across all SVHs in accordance with BVH standard practices. Develops, recommends, and implements policies, procedures, new tools, and strategies to drive positive QAPI related clinical outcomes within accepted standards of care.

15% Onsite Visits: Conducts regular onsite visits to SVHs to assess policies and procedures, resident medical records, care plans, MDS submission, medication error rates, wound reports, infection trends, restorative nursing programs, and QAPI program. Attends and contributes to meetings at SVHs homes including but not limited to change of shift report, care plans, interdisciplinary team (IDT), fall reduction, quality assurance and improvement, behavior management, infection prevention and control, restorative nursing, wound management, abuse prevention, and other meetings as requested by the SVH senior management. Supports the SVH as needed by participating in activities such as interviews for new hires, training schedules, staffing schedules, survey readiness, plans of correction, and professional development.

10% Onsite Audits: Performs regularly scheduled and unannounced inspections of all areas/disciplines to assist in correcting deficiencies and prepare for any agency inspections supporting the six SVHs. Provides regular reports to the QAPI Administrator on status of current activities, problem resolutions, planned correction for anticipated or current deficiencies, and general information related to accomplishment of BVH QAPI program goals. Develops and monitors methodology for evaluating program practices as applied to delivery of clinical and non-clinical services of the facility. Collaborates with BVH Compliance Officer, BVH Privacy Officer, BVH Clinical Services Administrator, DMVA Training Officer, and SVH IDT, including the QAPI & RNAC staff.

10% Consulting: Consults with nursing leadership, registered nurse assessment coordinators, and quality management at SVHs regarding clinical services including, but not limited to, resident care, clinical competencies, policies and procedures, regulatory compliance, training curricula, QAPI programs, quality measures, and Minimum Data Set (MDS) Assessments.

5% Mentorship: Provides consultation and professional mentorship to Clinical Service Managers (DON), Assistant Directors of Nursing (ADON), RN Supervisors (RNS), Infection Preventionists (IP), RN Instructors (RNI), and Registered Nurse Assessment Coordinators (RNAC).

5% Professional Development: Meets on a regular basis with QAPI administrator to discuss professional and personal development, QAPI program goals, DMVA/BVH safety hazards, workplace issues, or facility needs. Remains current with practices and regulations by reading, attending seminars, conferences, and webinars. Researches and employs evidence-based best practices for improved clinical outcomes.

Maintains positive, constructive working relationships with co-workers; remains aware of the needs and sensitivities of others. Conducts job duties in compliance with established BVH and SVH policies, guidelines, and procedures.

Maintains and improves job skills and competencies by participating in and attending staff training programs in order to improve the quality of resident care.

Performs work in accordance with the DMVA general safety rules. Immediately reports all workplace injuries or accidents to the supervisor; and/or immediately corrects any safety hazards in the work area or reports same to the supervisor.

Attends all assigned QAPI training. Follows established policies and procedures in support of QAPI efforts. Supports QAPI efforts both verbally and non-verbally, including adjusting performance and practice in accordance with QAPI initiatives and findings.

Follows standard precautions. Uses required personal protective equipment such as gloves, masks, gowns, goggles, face shield, hearing protection, foot protection, etc.

Performs other related work duties as required.

**Obtains and maintains Resident Assessment Coordinator - Certified (RAC-CT) Credentials. Maintains proficiency and knowledge of the current version of the Minimum Data Set (MDS) 3.0.

Periodic conferences are held with the QAPI Administrator to provide updated information, discuss significant projects and initiatives, and develop new goals and strategies related to professional nursing services and QAPI program goals.
