Position Purpose:
Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. 

Perform duties of a Licensed Practical Nurse in planning, implementing and evaluating all assigned resident care in collaboration with a Registered Nurse in a Department of Military and Veterans Affairs veterans home ensuring adherence to HIPAA standards.

10 %Assist the RN in the daily assessment of resident’s health status. Ensure all assessment data is accurate, complete and accessible. Communicate assessment data to other members of the health care team and preserve confidentiality.

40% Administer medications and treatments timely and accurately as ordered by a physician. Order and receive medication per procedure. Count narcotics and verify that records are accurate and complete. Check medications from pharmacy for accuracy and notify pharmacy of errors.

5% Participate in feeding residents as part of the daily routine and communicate concerns to RN in charge.

5% Receive reports from off-going shift and give report to on-coming shift as needed.

10% Make rounds and verifies resident census to ensure resident well being and safety.

5% Collaborate with RN in developing and formulating plans of resident care by focusing on identification of resident problems and resident outcomes. Implement planned changes to improve resident care as directed. Serve on nursing committees as necessary.

5% Observe residents’ conditions; ensure changes are reported to appropriate staff. Record daily care performed for the residents on the appropriate forms and the approved electronic medical record. Participate in documentation of resident admission data. Complete consultation sheets, record appointments and chart medications and treatments according to procedure. Forward Medical Record or abstract to facility upon resident transfer.

4%Assist with orientation of LPN and CNA. Monitor/direct CNA performance of health related duties.

2% Observe residents for injury. Notify the RN Supervisor if possible injury is observed and record data on resident medical record.

2% Ensure residents are clean and well groomed. Observe resident care in dining room.

2% Provide specialized care to residents on specialty units (i.e., Alzheimer residents).

1% Inform staff when residents cannot participate in activities and when they can resume activities.

1%Monitor supplies of necessary materials for nursing care and inform RN/Nurse Supervisor of needs.

1% Follow Infection Control guidelines.

1% Evaluate orientation program, Resident Care Plans, resident policies and procedures, and nursing policies. Recommend changes and attend various meetings as necessary.

1% Attend conferences, meetings, and seminars as well as all mandatory in-service training programs as part of ongoing education. Satisfactorily complete all required competencies.

1% Night-shift LPN: Restock documentation sheets in the chart as needed. Review physician orders every night to ensure that all orders were transcribed accurately into the electronic medical record and/or paper chart to include the treatment record, the medication administration record, and any appropriate communication record (e.g. 24-hour report sheet).

1% Review physician’s orders on all transfers of a resident intra-facility and upon return from a community hospital.

1% Support the facility QAPI efforts by attending all assigned QAPI training and following established policies and procedures in support of QAPI. Participate in PIP teams as assigned. Share ideas for improvement in the organization; support QAPI efforts both verbally and non-verbally, including adjusting performance and practice in accordance with QAPI initiatives and findings.

1% Performs work in accordance with the DMVA general safety rules and safety rules for this position. Immediately reports all workplace injuries or incidents to the supervisor; and/or immediately corrects any safety hazards in the work area or reports same to supervisor.

1% Perform other related duties.
