Position Purpose:
Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. 

This position has primary responsibility for the coordination and implementation of transitional services at Southeastern Youth Development Center (SYDC). More specifically, the position ensures that the facility successfully transitions youth back into their community through leading an active process of discharge planning and establishing aftercare via a network of community-based resources.

An employee in this position is also responsible for providing specialized, professional counseling and care for the rehabilitation and social development of court committed adjudicated delinquent residents in the Youth Development Center/Youth Forestry Camp (YDC/YFC) system. Supervises the resident group in rehabilitative treatment, education, recreation and social development programs in accordance with the Department of Human Services, Office of Children Youth and Families, Bureau of Juvenile Justice Services Programs and agency policies.

Description of Duties:

Provide security by maintaining constant vigilance through the direct supervision of residents in order to provide a safe living environment, which includes the prevention of harmful behaviors by residents towards self or others.

1. Ensure the safety and security of employees, residents and the community by preventing residents from absconding.

2. Maintain order and discipline by intervening with residents through active supervision, responding to emergencies and employing the least restrictive intervention technique in order to prevent escapes, control inappropriate behavior and maintain a safe and secure environment.

3. Search residents as well as all areas for contraband.

4. Escort and supervise residents to and from various activities, clinics, appointments and visits on and off grounds.

5. Know, follow and support all Bureau of Juvenile Justice Service’s and facility policies, procedures, philosophy and guidelines.

6. Adhere to all federal, state and agency regulations, policies and procedures governing confidentiality.

7. Adhere to all federal, state and agency regulations, policies and procedures governing the Health Insurance Portability and Accountability Act (HIPAA).

8. Provide instruction and guidance to residents in developing (1) proper and acceptable behavior and (2) competencies in their daily activities to include but not limited to: pro social skills, moral reasoning, academic, workforce development skills and job training, independent living skills and life skills.

9. Provide guidance in social and emotional growth by role modeling pro-social values and ideals in order to provide insight into more responsible behaviors to elicit a genuine desire to change.

10. Apply the principles of Balanced and Restorative Justice (BARJ) and consider these principles during the development of case management plans and other work situations as required.

11. Prepare, submit incident reports through the Juvenile Justice Automated Case System (JJACS).
12. Actively participate in and support the program’s multidisciplinary process and the overall Master Case Planning System. This includes assuming responsibility for the aftercare/discharge portions of the related documents.

13. With the understanding that aftercare planning begins at referral, initiate and maintain a focus on aftercare and transitional planning with the resident’s parent(s)/guardian(s), family, probation officer and facility staff.

14. Through the use of presumptive eligibility and/or working with a resident’s private insurance carrier, ensure that youth have access to and funding for medical and behavioral health services upon release.

15. Identify the needs of the youth, their family and/or social support system. This process shall include, but not be limited to, the use of normed/validated risk and need assessments.

16. Ensure that a Youth Level of Service Inventory (YLS) is completed within the required timeframes specified by the PA JJSES YLS Subcommittee and entered into JJACS within specified timeframes.

17. Facilitate the development and implementation of Competency Development and Treatment groups related to the transitional issues of youth at the facility.

18. Provide specialized individual counseling to residents as needed.

19. Document residents’ progress in groups and individual counseling in JJACS.

20. Support the youth and facility through participating in court hearings as determined necessary and appropriate.

21. Support Performance Based Standards through collecting and maintaining relevant data and participating in related facility-based initiatives, which includes administering the PbS Youth Exit Interview two weeks prior to the anticipated discharge hearing.

22. Coordinate with outside agencies and establish a resource network that supports youth leaving the YDC/YFC system.

23. Primary responsibility for the facility’s overall aftercare/discharge planning process, to include but not limited to, establishing follow up appointment dates and times for community-based resources.

24. Support programs and youth through initiating and completing benefit paperwork in cases of resident inpatient hospitalization.

25. Facilitate the identification, connection to and funding of appropriate academic, career and vocational programs for youth upon release.

26. Create and maintain a “Discharge Portfolio” for youth leaving the facility.

27. Facilitate the process by which residents receive copies of relevant vital documents, including birth certificates, social security cards, State ID and Driver’s Permit.

28. Conduct follow up when directed to ascertain if a youth is participating in their transitional/aftercare plan or if additional supports need to be developed.

29. Attend and participate in staff meetings, supervisory conferences and employee training programs, including but not limited to Crisis Intervention/Suicide Prevention, Safe Crisis Management, CPR/First Aid, Cultural Diversity and counseling-oriented trainings, including but not limited to: general counseling, substance abuse, mental health and/or developmental disabilities, in order to develop and maintain position skills.

30. Conduct a transitional orientation with the resident, individually or as a group.

31. Ensure copies of produced or procured documents are attached within JJACS.

32. Obtain standard releases of information for transition service providers (Parent/guardian, School, psychiatrist, JPO, State Court Unit, MH/MR, D&A, etc.) and attach in JJACS.

33. Develop discharge plans for youth in the event of an unexpected discharge.

34. Provide trainings to employees, youth and individuals outside of the YDC/YFC system as determined appropriate.

35. Represent the Bureau of Juvenile Justice Services at meetings and committees as determined appropriate.

36. Facilitate Counseling Center groups when directed to avoid cancelled/rescheduled groups.

37. Assume caseload responsibilities for program YDCs who may be out for extended periods.

38. Provide program coverage.

39. Complete Pennsylvania Academic/Career & Technical Training Alliance (PACTT) requirements.

40. Complete required components of 2nd Chance Grant programs with the counties.

41. Work overtime as required by operational necessities and emergencies.

42. Identifies, interprets and implements best practice standards.

43. Perform related duties as required.
